If your child receives a physical, a new vaccine, has been diagnosed with an illness, is going to be absent from school, is taking new medication during school or at home - please contact me by telephone or e-mail.
Remember, any medication, whether prescription or over the counter, requires a medication administration form to be completed and signed by both parent or guardian AND a doctor. Most forms can be downloaded off the nurse website.
September is bustling in my office. All students are weighed, measured, BMI calculated, blood pressures checked, vision checked in the 6th and 8th graders, hearing checked in the 7th graders, scoliosis checked in the 5th and 7th graders. All results will be recorded on a worksheet and sent home.
If your child needs to take medication during school please drop the completed and signed forms and labeled medications to my office as soon as possible.
If your child has had any change in health over the summer ie; surgery, new medical condition, new medications, please contact me to discuss them.
I am here to assist your child maintain his or her health while attending school. Please help me to assure your child is healthy by updating me as changes in health occur.
